Use this service to get your first sick (fit) note for work following an absence of more than 7 days.
If you require a sick (fit) note related to a hospital procedure such as an operation, contact the doctor responsible for your care. Hospital doctors are required to provide sick notes in this instance.
You can use this service if you:
- are registered at the surgery
- have not yet had a sick (fit) note for this absence
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
