Medical report and letters request

Use this service to request a medical report or letter.

You can use this service if you:

  • are registered at the surgery

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of

Asking for copies of your existing medical records must be done by attending the surgery in person with photo identification. Such a request counts as a Subject Access Request (SAR) under the EU General Data Protection Regulation (GDPR) and serves to protect the information we hold about you.
