If you would like to request a medical report or letter from your doctor, please use this form. There may be a charge for this service as the work is often not part of the core NHS work. It usually takes 5-10 working days before your letter or report is ready for collection.

Please note that asking for copies of your existing medical records must be done by attending the surgery in person with photo identification. Such a request counts as a “Subject Access Request” under the EU General Data Protection Regulation (GDPR) and serves to protect the information we hold about you.
